FAN MOSAICS, LLC TERMS & CONDITIONS

By submitting a photograph or other graphic reproduction to Fan Mosaics, LLC ("FM") you agree to the following terms and conditions:

1. You certify that you are the sole owner of the image/material you are submitting to FM and you certify that everyone included in this photo is of age 21 or greater. If not, this photo will not be included in the making of the Fan Mosaic.
2. You acknowledge you are submitting material which may be incorporated into a creative work and which may thereafter be sold;
3. In consideration for possible inclusion in an FM product, you irrevocably transfer and assign all rights to and interest in the material submitted to FM;
4. You agree FM may reproduce; distribute; publish; display; edit; modify; create derivative works and otherwise use the material for any purpose in any form and on any media without paying you any compensation whatsoever including, without limitation, licensing or royalty fees;
5. You represent that all persons depicted in the submitted material have given their consent to have the material submitted to FM and have agreed to be bound by these Terms & Conditions; and
6. You agree FM is relying on your representations contained in these Terms & Conditions and agree to indemnify FM for all damages and expenses (including attorney's fees) FM incurs as a direct or indirect of its use of the material submitted by you and/or its reliance on your agreement to these Terms & Conditions.

Photo Submission Guidelines:

Fan Mosaics, LLC ("FM") monitors every photo submitted to ensure each limited-edition FM print is "family friendly." FM reserves the right to remove any image it (in its sole discretion) deems inappropriate and/or which falls into any of the following categories:

• Contains sexually explicit or suggestive material, depictions of violence or is derogatory towards any ethnicity, race, gender, or religion;
• Promotes alcohol, illegal drugs, tobacco and/or weapons (or the use of any of the foregoing), any activities that may appear unsafe or dangerous, or any particular political agenda or message;
• Is obscene or generally offensive, or which endorses any form of hate or hate group;
• It defames, misrepresents or contains disparaging remarks about other people, products and/or companies;
• It contains trademarks, logos or trade dress owned by others; advertises or promotes any brand or product of any kind; or contains personal information such as license plate numbers, personal names, email addresses or street addresses;
• It contains copyrighted materials owned by others, such as photographs, sculptures, paintings or images published on websites, television, movies or other media); or
• It communicates messages or images inconsistent with the positive images and/or goodwill with which FM wishes to associate.

In the event FM deems an image inappropriate, the person submitting the inappropriate image will be given the opportunity to submit an alternate image. All submitted material will be retained for one (1) year, after which it will be destroyed.
